This Privacy Policy describes how Attain Miami (“Attain Miami,” “we,” “us,” or “our”), a registered fictitious name (DBA) of COTE Services LLC, a Florida limited liability company, collects, uses, and shares information when you visit attainmiami.com (the “Site”). By using the Site, you agree to the practices described in this Privacy Policy.

1. Important Notice: This Website Does Not Collect Patient Health Information

We do not collect, store, or process any patient medical data through this Site.

This Site is an informational and marketing website only. It is not a secure channel for transmitting medical information, and it is not intended to be used to communicate about your health, symptoms, medications, treatment history, or any other medical matters.

Please do not submit any health or medical information through this Site or by email. If you need to discuss medical questions, please do so during your consultation or appointment, or contact our office by phone.

Health information you provide to us in person, during a consultation, or through our patient management and booking platform as part of receiving services is handled separately and is protected as confidential patient records under applicable law, including Florida’s patient records confidentiality statute (Fla. Stat. § 456.057). Information about how your patient records are maintained, and your rights regarding them, is available from our office upon request.

2. Information We Collect Through the Site

Because we do not collect patient data on this Site, the information we collect is limited to the following:

a. Booking Information (Collected by Aesthetic Record)

When you book an appointment, you will use an embedded booking form or link provided by Aesthetic Record, our third-party practice management and scheduling platform. Any information you enter into that form — such as your name, contact information, and appointment details — is collected and processed by Aesthetic Record, not by this Site.

Aesthetic Record’s handling of your information is governed by its own privacy policy and terms, available on the Aesthetic Record website. Information collected in connection with your care through Aesthetic Record is maintained as part of your confidential patient record and protected as described in Section 1 above.

b. Analytics and Usage Data

We use Google Analytics to understand how visitors use the Site. Google Analytics collects information such as:

  • Your IP address (which may be truncated or anonymized)
  • Browser type, device type, and operating system
  • Pages visited, time spent on pages, and links clicked
  • Referring website or search terms
  • General geographic location (city/region level)

This information is collected through cookies and similar technologies and is aggregated and used for statistical purposes. It is not used to identify you personally. We retain information collected through the Site only as long as needed for the purposes described in this Privacy Policy, unless a longer retention period is required by law. Analytics data is retained according to our Google Analytics settings. Patient records are retained in accordance with Florida medical record retention requirements.

8. Your Choices and Rights

You may:

  • Opt out of analytics using the Google Analytics opt-out tools described above or by adjusting your cookie settings
  • Opt out of marketing at any time (reply STOP to texts; click unsubscribe in emails)
  • Request access to, correction of, or deletion of personal information we hold about you by emailing compliance@attainmiami.com

We will respond to verified requests as required by applicable law. Note that requests concerning your patient records are governed by Florida patient records law and should be directed to our office.

9. Security

We use reasonable administrative and technical safeguards to protect information collected through the Site. However, no method of transmission over the internet is completely secure, and we cannot guarantee absolute security. This is one more reason not to transmit sensitive or medical information through the Site.

10. Children’s Privacy

The Site and our services are intended for individuals 18 years of age or older. We do not knowingly collect personal information from anyone under 18. If you believe a minor has provided us personal information through the Site, please contact us at compliance@attainmiami.com and we will delete it.

The Site may contain links to third-party websites and services, including Aesthetic Record, social media platforms, and others. We are not responsible for the privacy practices of those third parties. We encourage you to review their privacy policies.

We may update this Privacy Policy from time to time. The “Last Updated” date at the top reflects the most recent revision. Material changes will be posted on this page. Your continued use of the Site after changes are posted constitutes acceptance of the revised policy.
